Why RN Case Management?
At Home Health Care, Inc. our primary concern is to promote the physical and emotional well being of our clients. In order to support this philosophy, Home Health Care, Inc. provides individualized RN Case Management for each client.
RN Case Management is a multifaceted approach to managing a client’s care that encompasses physical assessments, skilled nursing interventions, medication review, communication with the client’s physician, management of special dietary needs, and the coordination of services from other agencies including physical, speech, and occupational therapy.
Upon intake of a new client the Registered Nurse will conduct a thorough assessment of the client’s physical status and pertinent medical history, and consult with the client’s physician to determine the client’s primary diagnosis and any secondary diagnoses that may impact their health.
The RN will obtain a list of all medications that the client is taking including over the counter medications and supplements, noting any potential side effects and drug interactions that may occur. The RN confirms with the physician that the client is on the correct dosages and medication regimen.
The RN will also evaluate the physical layout and psychosocial environment in the home, or facility, where the client resides. One key element of supporting client well being is ensuring that their living environment is safe and facilitates their ability to age with grace and dignity. The RN will assess the need for additional safety equipment and seek orders for that equipment from the physician.
It is our belief that all people should live their lives to the fullest. Therefore, we also attempt to provide stimulating and interesting activities for our clients using their individual interests as a guide. Activities may include taking the client to an adult day center, geriatric picnic, or meeting, or just playing cards and providing companionship.
Based upon the compiled information the RN will develop an individualized plan of care for the client and forward it to the physician for approval. The plan of care is implemented by the RN in cooperation with an appropriately trained caregiver, and is subject to ongoing RN oversight and review. Ultimately it is up to the client to embrace and utilize the plan to it’s fullest potential- we provide education as needed to support the client’s readiness to accept their individualized plan of care.
Home Health Care, Inc. provides RN Case Management for clients living at home or in facilities. The RN Case Manager fills many roles: nurse, social worker, and friend, while coordinating the activities among all members of the team, including physicians, therapists, family and client.
